Have you got SPB Mobile shell 3.5 on your device?

Solved it. It's the activesync connection setting - allow wireless connection on device  when connected to the desktop.

I did.  I uninstalled Activesync - and reinstalled (4.5).  My icon reappeared and I've been able to use.
