Running scripts at startup

OK, I've tried /system/etc/init.d, creating an init.rc that creates a mount point and have searched the internet for anything that will point me in the right direction and have come up trumps.

All of these using custom ROM's that have the files in the images (though the init.rc would not install itself in root by adding it to the root of the custom zip I baked up).

I am stymied.

Does anyone know what to do to get a script to run at startup?
