Most Frequently Called/Received Phone Numbers Application

I'm looking for an application that maintains a prioritized list of phone numbers based on either called numbers, received numbers or both. The list would preferably be weighted by time (5 calls to a number in the preceeding week is higher on the list than 10 calls over the last 6 weeks).

Anybody got any ideas ?
