Voice Tag problem

Is there a fix for a voice tag orphans. I have some voice tags that are not associated with any numbers (I am not sure how this happened, but it has happened several times now).

When I speak the name I hear the orphaned recording play back and the number does not dial. I deleted the speed calls, through speed dial and deleted them through the resource manager. However, when I speak the orphaned name, the recording still plays back.

The only way I have been able to fix the problem is to do a HW reset. However, the problem is happening about every day now with different names.
