what are they and how do you install/use them

They are caller id themes.

You install them in the storage > My Docs > caller id folder and associate them with a caller id picture.

Look in the Orange folder in your start menu.

Yep, you can pick both a theme and a photo and then associate it with one of your contacts or a group of contacts ;)
