Is my phone dying?

For the past month or so, my phone has been getting less than a day of battery time, that includes no calling,gaming, or anything. Also, whenever my battery completely gets drained and shuts off automatically, the outer screen becomes a little more destorted. At first it was a line or two that was distorted, then chunks of the screen became distorted, and now its to the point where the screen is completely unreadable. Im guessing that the screen is gone but is there anything that can be done?
