Custom Phone Screen

I'm looking for specific software and can't seem to find it. Please let me know if you know of any software that replaces the tradition phone screen that you stare at after taking a call with a screen that shows the detail of the caller. Basically what I want is when someone calls me I take the call and immediately am looking at his/her contact details including all notes. I've tried a few and can't find something like this.
