WTB: Record GPS co-ord and some user-selected location info

One of our staff is after a simple application to record small bits of information, and include the GPS co-ords of where he is.

He walks around checking signs (and I thought my job was dull) and needs to record the type of sign, it's condition etc and the location of the sign (from his Bluetooth GPS device).

Does anyone know any apps that let you record GPS and user-specified info like this to a CSV file (or similar)?

You can get PDAs that will overlay GPS data on the photographs they take. Would this not be a better way of logging what type of sign, condition and location all in one convenient photo? Admittedly it may be harder to search than a csv file.
