Is this normal?

If I hold the phone in my right hand with the chin under my right thumb the screen rotates horizontally. This is normal.

If I hold the Hero in my left hand with the "chin" under my left thumb the screen does not rotate at all.

I didn't notice this before but is it by design?

It's contradictory also because for a right-handed user the back button is difficult to push with the thumb on your right hand.

Seems they made the phone for right-handed people because it won't flip the screen for left-handed users ... yet they made it difficult to use for right-handed people because of the position of the back button. The back button is probably the button I push most on the phone.

How the heck do you have trouble hitting back in landscape mode?

Actually, this is an issue at Google code, issue 3431. You can vote for it here. Perhaps when there are enough votes they will change it.

No difficulty in landscape. I have trouble bending my right thumb to hit back when in portrait mode though.

I Don't see where would be the inconsistency to display the same image at 180°.

And I really don't understand why this would be a great challenge to do the modification.

It's not as this have to be coded from scratch... It's just a variable value somewhere !

Android can do great stuffs like open GL rendering it can't achieve to rotate a picture - because where just talking of rendering a sum of pixels on a device - 90 underclockwise.

To be left hand is a handicap in Android world? Maybe all the Google developers are right hand ?

There is no reason (for me) to restrict that rotation. Maybe yes if somebody tells me that this will drain the battery quicker or brick the phone LOL.

iPhone do it... We can do it better

Cheers. I've just starred it. Iirc, there's already a kernel patch to fix this - I think it's in some custom roms or will be soon. I might have caught this on a Cyanogen thread.
