Anyones Sanfrancisco keep turning itself down?

I don't know how mines managing this but i'm setting it to loud and it's seeming to be turning itself onto the lowest ringtone/notification tone and i'm missing calls and texts.

I've installed audion manager and volume locker from the market which means any changes to the volume have to be confirmed before they can happen but i'm still finding the phone going very quiet on me. :unsure:

Anyone else having these issues? I basically either want it loud or silent!
