Lost apn after de-oranging

Hi beginner here I have tried to search but none of the settings I found seem to work but could be me doing it wrong.

Ok unlocked and deoranged last night but have wiped the apns, everything else fine internet through wifi and make and recieve calls.

When I go to add apns everything blank ie all setting say "not set"

Anyone got the full T-mobile settings.
