Here is my own mod.

It was tested in ModacoCustomRom R12, 13, 14, 15

Please always do a nandroid backup before flashing.

Installation : apply zip in CWM recovery

Installation

• Download the zip file of your choice from the links below and copy to your SD card (/sdcard/_ExternalSD)
  
• Restart your device in recovery mode
  
• Perform a nandroid backup (optional but recommended)
  
• Select the option to apply an update zip, and select the zip file you copied to your memory card

Can you please provide some tutorials about how can we make our own mod?

I'm interested to port this to my another phone.

Yes no music control in statusbar

Tried a couple of the mods and ended up using yours. I dont mind missing the Music Controls, but the other control bar are quite convenient.
