Help With AppInventor

Noob to developing Android apps here but prev done VB etc way back in college, the boss lady asked me to create a "simple app" with a box to input a team name, a box to input session type and a list to select a score from 1-5 and a submit button.

This info would need to be saved locally in csv/excel/access format, does anyone have any guides that would help me get this rattled up quite quickly ??
