"Phone" icon vanished

A couple of days ago the "phone" icon vanished from my Blade - any ideas please?

I had a similar issue but I lost App launcher on home page. I had somehow deleted icon from hidden dock (post 9033 onwards ) and had problems getting it back. With help from here last night I eventually got it back by creating a new shortcut and dragging it to dock.

You're a star - thank you

I had to hold down the spot where phone was and choose "swipe gesture" when given Edit Dock Shortcut option. I also had to Change Icon and select the phone.
