Auto unwanted call blocker

I'm after an application that will automatically block withheld/private numbers directly to voicemail. Ideally it will still give me some kind of silent notification that someone called.

Also, I need the application to support a black list of numbers (and being able to add a note against each number in the black list).

Can anyone suggest this? Thanks very much :-)
