SD Card Fail.

i mounted my sd card to my pc but came up with errors, saying that there was files in there that wernt reconized and i tried to Format it, but i keep getting error, it says there is a problem

i dont know what to do anymore, i need my sd card

i tried formatting it on the android 2.1 device but still no luck when i connect it to the computer

im stuck for what to do

can anyone help please?
