Just install https://play.google.com/store/apps/details?id=rubbebigpepper.VoiceRecorder and make sure you use the default settings.

thanXX redhawkuk.. 'll definetly try this one.. but in any way of modding phone.apk to add some extra facility.. ? like voice changer :D

It's not possible to hack the audio feed while in call mode however you could use a voice changer kit from ebay http://www.ebay.co.uk/sch/phone+voice+changer.

The sound quality won't be particularly great (real-time pitch bending kits never are) but good enough to hide / obscure your true voice.
