Did you uninstall the previous theme using Settings, Add / Remove Prgrams before loading the new one ? You could try reinstalling the theme and before you change to something else, uninstall it. If you look in there, is there an entry for SmartTweak ? That will be the extra screens. Otherwise a reboot of the phone should sort you out.
