This must have been covered somewhere, but I don't see the exact problem specified or a fix. Any ring I set on my O2 or my girlfriends, starts off quiet and then increases to full volume over a period of a couple seconds and stays that volume for the remainder of the call. This is regardless of what volume I have my phone set at before the call comes, and I do not have increasing ring selected in ring settings.

This is not the issue where I want to stop it from being an increasing ring necessarily, I just want to be able to set my ring volume. It's ok if it increases.. but it shouldnt increase to full volume (so the neighboring cities can hear it) no matter what my volume is.

How do I fix it?

You have to set your Ringtone to max level. (and you can set the system volume to a lower level if you like)

What do you mean? Where do I set the level of the ringtone? I don't want the ringtone at max level, I want it quieter.

Ah, I misread your post thinking you wante max Ringvolume and lower Systemvolume but I guess you want it the opposite way?

Anyway the Ring and System volume can be set separately. Just access the Samsung Volume control (pressing on the taskbar or the volume up/down scroller).

Or maybe I don't understand your question exactly
