Two faults with my new Pulse Mini

I have a new Pulse mini which has two problems.

1. I frequently get a message saying the SD card is not mounted. I have to take it out and put it back to make it work. T-Mobile said there is too much room in the slot which allows the card to move up and down. They suggested either send it back (could take weeks) or sticking a piece of paper in the slot to keep the card in place. Has anyone else has this problem, how did you fix it?

2. When I get a missed call, the on-screen message says it came from the first name in my contacts list. (i.e. Missed call from Alan) I know that this is wrong. I tested it a couple of times from different numbers and it does it every time. Any clues please.
