not recognizing SD-Card after installing Custom ROm

Hi there..

 

I've recently installed LiquidSmooth Custom Rom,it were going right,but after awhile it showed (unexpectly removed Sd) and immidiately (preparing sd) it continued again and again .

 

now it no longer shows my sd card ..it works fine when I connect Sd card to the computer ...

I flashed Cyanodgen custome rom .. again it was good ..after awhile it showed the same problem .now it doesn't show my SD-card anymore...

 

I know it's not the physical problems ..because this issue happened after flashing a new custom Rom ..

I've already formatted but it doesn't mount the sd-card..it works well in other devices but not recognize it in my phone

guys please help me to solve this problem 

what to do ...?
